(2) What is the procedure to be followed by the ESI beneficiaries to avail Super Speciality Treatment?

The beneficiary should approach his dispensary for reference.  Reference will be issued for cases for which treatment or investigation facility is not available at ESI institutions.  Insurance Medical Officer will refer the cases to the nearest ESI Hospital from where the Superintendent will give reference letter to the empanelled centres of IP’s choice.  If there is no ESI Hospital within 25 KMs of the dispensary,the Insurance Medical Officer himself will refer the patient to the empanelled centre.

Documents required:

  1. Application of the IP
  2. Entitlement Certificate with Sickness Benefit (original + 3 attested copies)
  3. Photo Identity of the patient duly authorized by Insurance Medical Officer
  4. Original ESI Card with three attested copies
